UNDERWARD thrusts players into the grim corridors of an abandoned underground asylum, tasking them with retrieving experimental monsters. It supports cooperative play for up to four, features cross-platform multiplayer, and relies heavily on real-time voice communication and quick reflexes to stave off an untimely demise.

Each run through the decaying asylum offers a procedurally different layout. Players must locate the experimental subjects—housed in culture capsules—and ferry them to the elevator. Dodging erratic nurse monsters and lethal traps is essential, but be warned: the game features a 'permadeath' system where a total party wipe resets your entire progress.

To speed up the extraction, players can utilize the 'linked transport' system, requiring at least two people to physically connect and haul the cargo together. Any tools or items scavenged during the exploration can be retained and used across different floors.
